Home Health Services

Community Health Services is a Medicare Certified Home Health Agency located at Syracuse Community Memorial Hospital.  Our agency provides home health services to Otoe, Cass, Johnson, Nemaha and Lancaster counties.  Our agency has dedicated RN’s, LPN’s and Home Health Aides who provide care to patients in their homes.  We also provide Occupational, Speech and Physical Therapy services to assist in your rehabilitation needs.  All services are provided under the direction of your physician.  Most skilled services provided in the home are covered by Medicare, Medicaid and Private Insurance payors.
